PRIVACY NOTICE AND DATA PROTECTION FOR PROSANTÉ CUIDADO INTEGRAL DE LA SALUD S.A. DE C.V. (HEREINAFTER "PROSANTÉ")
PROSANTÉ, in its capacity as the "Controller," with a legal address at Av. Paseo de la Reforma #404 Floor 1-B, Colonia Juárez, Cuauhtémoc, Mexico City, C.P. 06600, issues this "Privacy Notice" in accordance with the provisions of Article 15 of the Federal Law on the Protection of Personal Data Held by Private Parties, in force as of July 6, 2010, hereinafter "The Law." The "Controller" guarantees the safeguarding and protection of the personal data of all its clients, who will be referred to individually as the "Data Subject," under the following terms:
A. Data Collected and Purpose:
We only collect information that you freely and voluntarily provide, physically, by phone, or electronically, which may include but is not limited to the following:
1. Personal Data: Full name, date of birth, email, work phone, home and/or cell phone, as well as identification or employment data. The primary purposes for collecting this data are identification, statistical management, appointment scheduling, and internal analysis. Secondary purposes include sending promotional information about PROSANTÉ services.
2. Sensitive Personal Data: Will not be collected.
The "Data Subject" may request access, rectification, cancellation, and opposition of their Personal and Sensitive Personal Data through the means specified in section D of this notice.
The "Controller" guarantees the protection and appropriate use of the information in accordance with "The Law." All personal data will be safeguarded at the highest security levels, taking administrative, technical, and physical measures to respect the "Data Subject" rights, as stipulated in Article 19 of "The Law."
PROSANTÉ is not authorized to share any personal data provided by the Data Subject or obtained by PROSANTÉ with any third party except in cases explicitly stated in this document and/or where the "Data Subject" has given their consent.
B. Storage and Disclosure:
We are committed to safeguarding your personal information, ensuring legal, technological, and administrative security, and not selling, renting, sharing, or disclosing your personal information to third parties for any purpose. There will be no commercial transfer of data, nor will they be used for purposes other than those mentioned above, except if a change in this Privacy Notice is made and the corresponding consent is obtained from the "Data Subject."
C. Rights:
Regarding personal data protection, PROSANTÉ guarantees respect for your rights (commonly known as ARCO Rights) through the communication means established in section D of this Privacy Notice:
1. Access: You may request access to your personal information that we hold at any time.
2. Rectification: You may request that we correct any of your data if it is incorrectly recorded.
3. Cancellation: You may request that we cancel or remove your data if you no longer wish to remain in our database.
4. Objection: The "Data Subject" may object to the processing of their personal data through the communication channels specified in this document.
D. Requests:
You may request the exercise of your ARCO rights mentioned in section C of this Privacy Notice at any time by submitting a written request, either physically or electronically, addressed to our offices (address detailed above) or via email. This request should be addressed to PROSANTÉ, with the electronic address being privacidad@prosante.com.mx, and should contain the following: name, address, copy or photograph of an official ID of the data subject, email (for communicating the response to your request), and a clear and precise description of the personal data for which one of the rights will be exercised, along with any other information that facilitates locating such data. You can revoke the consent you've given us to process your personal data at any time so that we stop using it. To do this, please submit your written request to the PROSANTÉ offices previously located or in an electronic request to privacidad@prosante.com.mx. This request should clearly express the information that should no longer be processed. The "Controller" will respond to your request within the next 72 business hours.
E. Amendments:
We reserve the right to change this Privacy Notice at any time. If there is a change, it will be published electronically and physically in the aforementioned offices.

F. Express Consent:
The "Data Subject" of personal data declares through this instrument that they consent to the "Controller" safeguarding and using all sensitive data collected for the provision of services for the primary purposes previously expressed.
This Privacy Notice complies with the requirements of the Federal Law on the Protection of Personal Data Held by Private Parties in Articles 8, 15, 16, 17, 18, and 36.
